Before a DSS Event

Speaker Request Form:  Use this form to request availability and/or book a speaker through ASID headquarters.

Host Responsibilities & Checklist:  Organization hosting DSS programs will be responsible for the execution of these tasks.

DSS Logo:  Please use this DSS logo in conjunction with all print or electronic event marketing.



During a DSS Event

Meeting Room Sign:  ASID chapters hosting DSS programs shall locally produce a 24x36 foamcore meeting room sign using this artwork.

Evaluation Form:  Distribute copies of this evaluation form onsite and collect them from learners at the end of the course.

Certificate of Completion:  Immediately after the course, distributed copies of this signed certificate to all participants as their immediate verification of successful completion. 

Attendee Sign-In Sheet:  Use this sample attendee sign-in sheet or create your own to capture this information during your event.

Registration List:  Use this template to capture your attendee information such as name, address, phone number, and email address during registration.



After a DSS Event

Event Summary:  Capture all the details about your event including registration statistics, pricing, evaluation results, comments etc. and return this information to ASID within 7 days of your event.
